Aleksander Kulisiewicz collection Polskie Wydawnictwo Muzyczne, PWM Questionnaires from Slaski Scientific Institute questionnaire.

Contains a 15 page questionnaire developed by the Slaski Scientific Institute used by Kulisiewicz to solicit camp songs from concentration camp survivors and to collect information on cultural life in the camp. Information retrieved and compiled from these responses was intended for Kulisiewicz's anthology, Polski Piesni Obozowe, 1939-1945. Also contains two alternative questionnaires written for the same purpose.

Aleksander (Alexander) Kulisiewicz (1918-1982) was born in Kraków, Poland in 1918. He was a law student in German-occupied Poland when, in October 1939, he was denounced for antifascist writings, arrested by the Gestapo, and sent to the Sachsenhausen concentration camp, near Berlin. An amateur singer and songwriter, Kulisiewicz composed 54 songs during more than five years of imprisonment at Sachsenhausen.
